This agreement collects your basic contact information and emergency information and the required liability waiver and studio policies for participation in classes, events, workshops, and other fitness activities held at Destiny Fitness. By signing this form you acknowledge that you have read, understand, and agree to the liability waiver and studio policies.

I agree to:

Follow instructor directions.
Wear appropriate athletic attire.
Respect studio equipment.
Return all equipment to its designated area after class .
Arrive on time.
Conduct myself respectfully toward staff and participants.
Understand that Destiny Fitness reserves the right to remove participants who fail to follow studio policies.

LIABILITY WAIVER

I understand exercise involves inherent risks including muscle soreness, strains, falls, equipment injury, and in rare cases serious injury.

I voluntarily choose to participate in training and use of equipment. I accept fill responsibility for my participation and agree to follow all trainer instructions.

I release and hold harmless Destiny Fitness LLC, it's owner(s), staff, instructors, guest instructors, studio renters, volunteers, and affiliates from any claims injuries, or damages arising from participation , including claims related to negligence.

I understand and agree to the liability waiver.

MEDICAL ACKNOWLEDGEMENT

I confirm I have disclosed all known health conditions and understand it is my responsibility to inform Destiny Fitness of any changes to my health. I understand Destiny Fitness and its instructors do not provide medical advice and participation is voluntary.

PHOTO CONSENT

I grant permission for Destiny Fitness to use photos or videos taken during training for social media and promotional purposes.

INDEPENDENT INSTRUCTOR ACKNOWLEDGEMENT

I understand that some classes held at Destiny Fitness may be independently instructed by guest instructors or studio renters. Regardless of the instructor, I acknowledge that I am voluntarily participating in activities held at Destiny Fitness and agree to the above liability waiver.

FINAL AGREEMENT

I confirm I have read and understand all policies and this waiver. I understand this a legally binding agreement with Destiny Fitness LLC and I agree to follow all studio rules
